Nancy, I suggest that you contact TriCare, explain your situation and they will be able to give you step-by-step instructions which will include forms to be completed by her legal guardians. From your email address it appears that you live in the west. If so, the TriCare number you should call is 1-888-TRIWEST. If you don't live in the western states, they will be able to give you the 800 number for your geographic area.

Thanks for your question.

My 6 yr old grandaughter is living with us while her Dad is in Iraq. Isn't she eligible for military benefits? (for ex. medical)If so how do I get her enrolled? We are not her legal guardians. Her other grandmother is but she is unable to care for her at this time. We live near Travis AFB.

Eligibility is based on a series of factors. As of October 1, 2003, unremarried former spouses became sponsors in their own right.

The Defense Enrollment Eligibility Reporting System (DEERS) now "reflects TRICARE eligibility for these beneficiaries using the unremarried former spouse's own SSN" and not the former sponsor's. Health care information is filed under the unremarried former spouse's own SSN and name. Unremarried former spouses now have to use their own SSN to set up a medical appointment and to file a claim.

The wife should contact the Defense Manpower Data Center Support (DMDC) Office to check her eligibility status in DEERS based on her legal separation. She can do this by visiting her nearest uniformed services ID card facility or by calling DMDC toll-free at 1-800-538-9552.

You should contact the customer service department of the TriCare division through which you are insured (such as TriCare Standard or Extra or Prime etc.). The phone number will be on your insurance do...entation. It is also on their website (the website link is in the above article). You can also stop by a TriCare office if you are close to a military installation.
