-- Contributed by: Tamsen ButlerOne great thing about guardian dental is the large network of dentists they have. We opted to go with a discount dental card instead, but seriously considered dental insurance.
Patty, you may want to ask your husband to speak to his HR department and obtain a full description of the benefits since this can vary from one employer to another. You may also contact Guardian directly: (212) 598-8000.

If you haven't already, I suggest that you send certified letters to both companies, explaining the issue. I suggest that you also copy the insurance commissioners of both Washington and Iowa as well as your employer.
-- Contributed by: SusanWeberI have contacted both the Guardian Group Dental Claims in Spokane, WA (800-541-7846) and Assurant Employee Benefits in Clinton, IA (800-442-7742) only to be told that I need to contact the other. Each company has denied any knowledge of the claim or the manner in which it was processed. I have exhausted my patience in this matter and hopefully will not submit any further claim on my Guardian Group Dental coverage.

To obtain answers about insurance issues you need to contact your insurance company directly. I suggest that you contact Guardian and ask if your claims were passed on to Assurant. The phone number for Guardian customer service is on your policy and on your claim forms or you can obtain the phone number from your employer.

I have confirmed and re-confirmed with my employer that it has NO association with Assurant, yet on two separate occasions (after having filed my claims with Guardian) I received notification from Assurant that the charges "were incurred after the patient's termination date of coverage". During my entire career, my employers have NEVER provided dental (or any other) coverage by Assurant. I am, therefore, led to believe that Guardian passed my claims on to Assurant for processing - how else would Assurant be involved in this scenario?

Perio-protect is FDA and ADA approved and is a fraction of what it costs to have other periodontal treatments. In addition it is painless and has AMAZING results. The main reason i decided to get the guardian dental insurance where i work was because the basic plus plan stated it will cover 90% of periodontal treatments/surgeries. I found out through my dentist that perio-protect IS NOT covered through guardian. I plan to drop the insurance in July.

Be sure to inform your employer that your dental benefit through Guardian is not a benefit to you since there are no dentists in your state. It is possible that Guardian can recommend a dentist in your state who they may be willing to work with through your coverage. You can start your inquiry by calling Guardian's customer service. If they are not able to refer a nearby dentist under your coverage, have your employer write a letter to Guardian for you.

You can search for a Guardian provider on their website or you can contact a Guardian office and speak directly with one of their representatives. The office addresses are available on the Guardian website. The contact information for your local office will also be listed on the policy information you received when you first set up your dental insurance policy.

Your enrollment packet should have phone numbers to contact them so you can find out exactly the difference in cost between in and out of network orthodontia. In addition, they can tell you their standards for reasonable distances to go to an in-network provider. You might also see if a closer orthodontist is willing to apply to Guardian to join their network.
