Standard Medical Release Form

From LoveToKnow Insurance

Schools and other organizations require a standard medical release form for each child who will be attending or participating in school activities, sports, or other functions.

Importance of a Standard Medical Release Form

This form authorizes medical professionals to administer medical treatment to your minor child in your absence.

Medical release forms serve three primary purposes: they prevent treatment delays if your child is injured or sick, they release healthcare workers from legal and financial liability, and they enable caretakers and teachers to authorize medical treatment for a child in their care.

Who Requires a Medical Release Form

Schools and other organizations generally require medical release forms before allowing children to attend school activities, participate in sports, or attend field trips. Many schools require a signed release form before allowing children to begin the school year.

Healthcare providers, including pediatricians, dentists, and other doctors, require signed medical release forms before administering medical treatment or prescribing medications, with the exception of life threatening medical treatments. Medical release forms protect healthcare providers from liability for any negative effects of the treatment rendered.

Additionally, relatives and other caretakers should keep a medical release form on file in order to obtain medical help for your child in the event of an injury or illness while your child is under their care.

Where to Find Medical Release Forms

Medical release forms are available at most schools, local health departments, and hospitals. Additionally, you can obtain a medical release form from your child's pediatrician.

Filling Out a Medical Release Form

Before filling out a medical release form for your child, read it carefully and gather any documents or other items you need. Most healthcare providers require the form to be filled out using blue or black ink.

  1. Name and contact information for both the child and at least one parent or legal guardian.
  2. Insurance information, including the name of the insurance carrier, the policy number, and the contact information for customer service and claims.
  3. Name and contact details for the child's pediatrician, dentist, and preferred hospital.
  4. Medical information, including any known allergies, current health conditions, prior surgeries, and a list of over-the-counter and prescription medications currently taken by the child.
  5. Emergency contact name, relationship, and contact information. Listing an additional emergency contact is important in case medical professionals are unable to reach you.
  6. Acceptance of financial responsibility and agreement to pay for any medical services rendered.
  7. Release of liability to protect the healthcare providers both financially and legally against claims you may file against them for medical treatments administered to your child.
  8. Signature of the parent or legal guardian authorizing medical treatment.

Considerations

To ensure your child’s medical release form is acceptable at all hospitals and other medical centers, fill it out completely using blue or black ink. Have the form notarized by a legal notary public, and save a photocopy of the form for your records.
