How Medical Release Blank Forms Work

If you're the coach of a school sports team, the leader of a Girl Scout troop, or heading up some other activity where there's a potential for any of the participants to get injured, it's important that you have the parents of those children fill out and sign a medical release form. Unless you're a lawyer it can be difficult to write the form by yourself from scratch, so using a medical release blank form can be a tremendous help.

Important Elements

A basic medical release form should:

  • Clearly identify the child or participant
  • Clearly state that you grant permission for medical intervention
  • Provide medical insurance information
  • Parental or guardian signature.

Certain sections of a blank medical release form should not be left blank. These are the sections that should make up the template of the form, and will remain the same on every form. These sections should be generic enough so that the form can be used for almost any group activity or sport.

  • Statement of permission such as, "I hereby grant permission for any licensed medical staff to provide necessary and reasonable medical attention to my child."
  • List of events that may require medical attention, such as "in the event of an injury, accident or illness."
  • Statement of financial responsibility such as, "I accept financial responsibility for the payment of any costs related to medical treatment."

The blank sections of the form should allow the form to be used for anything. These sections will have a label with a blank line to the right so that the person using the form can simply fill in the blanks.

  • The name of the responsible guardian
  • The name of the child or participant
  • The address of the responsible guardian
  • A list of emergency contacts if the primary guardian can't be reached
  • A list of responsible leaders of the event or activity with permission to act on behalf of the guardian
  • Insurance company name and policy number
  • The name of the participant's doctor including address and/or phone number
  • A list of all known allergies
  • A dated signature of the parent or guardian

While you could potentially use the guide above to put together your own form from scratch, it isn't necessary since there are so many free sources for these forms throughout the Internet. The following list provides a few examples of excellent free sources for these blank forms.

Preparation is Key

No matter what activity or sporting event you're planning, it's always a good idea to have every participant fill out and sign a medical release form. If you don't have these forms with you during the activity and someone gets hurt you could find yourself in a very difficult situation. Without a child's basic medical information - and permission from a parent or guardian to provide medical attention - it can be very difficult for medical personnel to take care of the injury or even save the child's life.
