Plan members must serve a three-month elimination period when they sign up for the Ameritas Access 1500. After that point, the Plan pays 100 percent of the rate for services listed in the Plan if the subscriber sees a dentist within the Ameritas network of providers. The covered services include dental examinations, teeth cleanings and topical fluoride treatments provided for children under the age of 19 years. Seeing a dentist who is not part of the Plan's network of providers means that subscribers will still be reimbursed at a rate of 80 percent for services rendered.
